Resumo: Objective: to evaluate histological features related to the progression of oral squamous cell carcinoma (OSCC) - tumor budding (TB), perineural invasion (PNI), depth of invasion (DOI), tumor thickness (TT), lymphovascular invasion (LVI), pattern of invasion (PI), tumor-stroma ratio (TSR), and tumor neutrophil-lymphocyte ratio (NLR). Study Design: Resection samples from 31 OSCC were included and immunostained with anti-pan-cytokeratin, CD66b (neutrophils), and CD8 (cytotoxic T cells). Results: TB was high in PNI and LVI-positive tumors and positively correlated with TT. Non-cohesive tumors frequently exhibited PNI, dispersed tumors exhibited the highest DOI and TT, and non-cohesive tumors presented higher CD66b at the invasive front than cohesive tumors. CD66b and NLR at the tumor center were associated with worse disease-free survival. Conclusion: Invasive cell phenotypes represented by high TB and non-cohesive/dispersed patterns were indicators of tumor aggressiveness related to PNI and LVI. Tumor NLR emerged as a potential prognostic marker, suggesting a pro-tumoral immune response associated with high neutrophil infiltration.
