Resumo: External cervical resorption has been described as another side effect related to denosumab. We present a case of a 39-year-old female patient complaining of spontaneous dental, lasting 7 days. She had a history of a breast cancer and has been treated with Exemestane and Denosumab for osteopenia, for 2 years, with an interval of 1 year during the treatment. Oral examination revealed good oral hygiene and areas of resorption on lower left mandibular first molar and right lower second premolar. Computed tomography presented a hypodense lesion to extends into coronal third of the root and apical to the bone crest. Endodontic urgency was performed on tooth #19, and curettage of granulation tissue within the defect. Histopathological exam presented areas of preserved and degenerated dentin. The diagnosis of external cervical resorption was established. Follow-up of patients undergoing antiresorptive therapies is essential to prevent adverse effects on the jaw bones and teeth.
