Resumo: A 60-year-old male patient with numbness complaint in the right side of the jaw. During examination, he reported a previous history of treatment of a lesion in the same region 10 years ago, with a diagnosis of odontogenic keratocyst. Intraoral exam showed a slight increase in volume in the retromolar region on the right side, along with a surgical scar on the gingiva. Image exams revealed the presence of a well-defined, circumscribed, radiolucent area located distally to tooth 48, above the mandibular canal. Based on the collected clinical information and image examination, it was treated as recidive with enucleation and curettage of the lesion, along with extraction of tooth 48, associated with chemical cauterization using 5-fluorouracil, without complications. The patient has been followed up for six months, with no complaints or signs of lesion recurrence. Complementary KO treatment is an easy and effective option in preventing recurrences.
