Resumo: A 5-year-old male patient with Down syndrome and congenital heart defect was referred to the specialized center with facial edema. The clinical diagnosis was dentoalveolar abscess. A mechanical chemical preparation of the root in the second primary molar was performed under physical restraint. Antibiotic therapy was indicated. The patient was referred to the hospital care where a panoramic radiography was performed. A radiolucent lesion in the mandible was identified and the medical team recommended a lesion's biopsy under general anesthesia. However due to pandemic the surgery wasn't performed. Eight months later the patient returned and no pain neither edema were seen. After 3 sessions of behavior management the extraction of the tooth and curettage of residual tissue was possible. Radicular cyst was diagnosed at histopathological exam. After 3 months of this procedure a complete bone repair was seen. The patient was referred for cardiac surgery for valve's disease correction.
