Resumo: A 33-year-old male patient attended a dental appointment presenting bulging of the vestibular and lingual cortical bone in the right posterior mandible, associated with symptomatology, with one year of evolution. The radiographic examination showed a well-defined unilocular radiolucent area with anteroposterior growth involving the mandibular third molar. We performed a fine needle aspiration and an incisional biopsy. We installed a decompression device which was removed after three months, obtaining important new bone formation with a reduction of the dimensions of the lesion. The anatomopathological analysis confirmed the diagnosis of plexiform unicystic ameloblastoma. The surgical treatment of the lesion consisted of extraction of tooth 48, lesion enucleation and curettage. After one year, we observed a lesion recurrence and performed a peripheral resection of the mandible, involving the right mandibular ramus and part of the coronoid process, without intercurrences. At the 4-month follow-up, the surgical wound presented complete closure without recurrence signs.
