Resumo: Dilated odontoma (DO) is the most extreme form of dens invaginatus. This dental anomaly is predominantly spherical in shape, which is why it is called odontoma "dilated". In this series, we describe three cases of DOs with an emphasis on imaging findings. All cases occurred in males with a mean age of 27.0 years (SD±4.5) and were asymptomatic. Two lesions were found in the mandible and one in the maxilla. Radiographically, the lesions showed an anomalous shape resembling a tooth crown, with a radiolucent central portion surrounded by radiopaque structures resembling dentin and enamel, enclosed by a thin radiolucent halo. Treatment consisted of conservative surgical excision. Morphologically, the lesions were composed predominantly of dentin and areas that resembled enamel matrix. The final diagnosis was DO. Careful imaging analysis is essential to ensure an accurate diagnosis and adequate treatment of this dental malformation preventing physiological, aesthetic, and functional problems.
