Resumo: Sarcomatoid Carcinoma (ScC) is a highly aggressive subtype of squamous cell carcinoma associated with a poor prognosis. Histologically, ScC has varying proportions of epithelial carcinomatous and mesenchymal sarcomatous components, making the diagnosis challenging. We present here three challenging cases of primary ScC. Patients were presented with ulcerated exophytic lesions in the lower lip (two) or hard palate (one) with no history of radiation therapy. Microscopic analysis revealed infiltrative malignant spindle cells interspersed with epithelioid giant and/or bizarre cells. Immunohistochemical showed variability in p63, vimentin, AE1/AE3, and CK14 expression. S100 was positive and αSMA was focally positive in one case. In conclusion, ScC is a lesion with a varied immunohistochemical profile and needs to be evaluated along with clinicopathological findings to be diagnosed correctly. Thus, ScC should be considered as a differential diagnosis when polypoid lesions with a morphological variation of malignant cells are presented in the lower lip.
