Resumo: Ameloblastoma is the most common odontogenic tumor, with slow and invasive growth, and can reach grotesque sizes. The patient ERS, 60 years old, underwent surgery for an ameloblastoma about +- 10 years ago, the lesion occupied a large part of the mandible, a partial resection and installation of a reconstruction plate was performed. About 6 months ago the patient presented a recurrence of the lesion, measuring in its largest diameter about 15 cm. After confirming the invasibility of the tissues through tomographic exams, a composite resection was performed, with rotation of a pectoral muscle flap to fill the defect. Anatomopathological analysis suggested ameloblastic carcinoma, but immunohistochemistry concluded that it was an atypical ameloblastoma, with no defining criteria for malignancy. The patient is being followed up by the Oral and Maxillofacial Surgery, Head and Neck Surgery and Oncology teams for flap maintenance and definition of possible new treatments.
