Título: FACTITIAL ULCER: REPORT OF AN ATYPICAL SEVERE CASE IN THE ORAL CAVITY

Resumo: A 24-year-old male patient, with history of epilepsy and the use of illicit drugs, was referred to the stomatology outpatient clinic for painful lesions in the oral cavity for about 3 months. The physical examination revealed multiple large deep ulcers located on lower and upper labial mucosa, tongue and peri labial skin region. During the clinical examination we observed that the patient was constantly biting the region. According to these characteristics, the main diagnostic hypotheses were infectious diseases, such as fungal and bacterial infections, and factitial ulcer. Incisional biopsies of the upper and lower labial mucous were performed and hematological exams were ordered. The microscopical examination showed a nonspecific ulcer with chronic inflammatory infiltrate and the hematological exams were normal, reinforcing the possibility of self-mutilation. The patient was medicated with anti-inflammatories and antibiotics and referred for psychiatric evaluation. In the follow-up we observed improvement of the lesions.
