Resumo: Brown tumor has been reported in around 3 times more female than male patients, usually in the sixth decade of life or older. A 20-year-old male was referred for evaluation of a mandibular painful mass, 2-months ago. Medical history he had chronic renal disease and had been on hemodialysis for 2-years. On intra-oral examination, a swelling of the lingual surface in the posterior region of the mandible was noted. The panoramic radiograph revealed irregular radiolucencies extending from tooth 34 to 48. An incisional biopsy was performed, and microscopy showed well-vascularized dense connective tissue, with variable numbers of spindle-shaped cells and diffusely distributed multinucleated giant cells. Laboratory tests evidenced high levels of PTH, calcium and alkaline phosphatase. So, a diagnosis of hyperparathyroidism brown tumor was established. The patient was treated for PTH control, and after 9-months of follow-up he is stable and showed a decrease in tumor size.
