Resumo: Sinonasal organizing hematomas (SOH) are rare, benign lesions that can be mistaken for malignant tumors due to their aggressive appearance on imaging. An 87-year-old female patient sought an oral medicine service complaining of left nasal obstruction. A CT scan revealed opacification of the right maxillary sinus, erosion of the right inferior orbital wall, and extension into the nasal cavity. An incisional biopsy was performed. Initial histopathological analysis was inconclusive, and the patient was submitted to surgical removal with peripheral osteotomy. The morphological analysis of the surgical specimen showed dilated vessels, hemorrhage, fibrin exudation, fibrosis, hyalinization, and neovascularization. Based on the imaging and histopathological aspects, the diagnosis was SOH. No signs of recurrence were observed after 15 months of follow-up. Due to the rarity and destructive nature of SOH, misdiagnosis and unnecessary aggressive surgical approaches can occur. Clinicians should consider SOH in the differential diagnosis of aggressive sinonasal lesions.
