Resumo: Erythema Multiforme (EM) is an acute inflammatory process, immune-mediated of mucocutaneous hypersensitivity. Male, 7 years old, presenting ulcerated lesions in labial mucosa, buccal mucosa and lateral border of tongue. Previously, he presented a Primary Herpetic Gingivostomatitis episode, with pain, fever, fatigue and lymphadenopathy. Later, he developed recurrent oral ulcerations, treated by medical staff with antibiotics and antivirals, without response to treatment. In stomatology evaluation, laboratory tests were requested, with normal results, Polymerase Chain Reaction (PCR) were negative for: Herpes Simplex virus (HSV- I, II), cytomegalovirus and Epstein-Barr virus. Incisional biopsy followed by histophatological analysis, revealed hyperplastic cells, with moderate inflammatory infiltrate. Direct immunofluorescence of tissue was negative for Pemphigus Vulgaris and Mucous Membrane Pemphigoid. The final diagnosis was EM secondary to HSV, correlating the clinical history and the manifestation of oral lesions. The lesions regressed with the use of corticosteroids and lysine and the patient was referred for dermatological monitoring.
