Resumo: Objective: To evaluate the clinicopathological characteristics, survival rate, and prognostic factors of primary intraosseous adenoid cystic carcinoma (IACC) in the jaws. Study Design: We conducted a literature search on MEDLINE/PubMed, Scopus, and Embase. The inclusion criteria were articles with IACC diagnosed in the jaws. Exclusion criteria included studies with insufficient demographic information, follow-up, and inconclusive histopathological diagnosis. Results: A total of 55 cases of IACC were reported among 27 studies. The mean age of diagnosis was 56.4±19.6 years, and the male-to-female ratio was 1:1.2. IACC typically presents as a slow-growing (36,4%), symptomatic mass (87.3%), with a significant incidence in the mandible (69.1%). Radiographically, IACC manifested as an ill-defined destructive lesion (40%), while histologically, the tumor exhibited a cribriform pattern in most cases (32.7%). Local recurrence was seen in 12.7%, while local and distant metastases were seen in 3.6% and 12.7% of cases. Solid tumors and distant metastases were predictive of poor disease-free survival (p=0.040; p=0.043). The 2- and 5-year overall survival rates were 57.9% and 53.8%, respectively. Conclusion: IACC affects middle-aged adults, with no sex preference. The most common site of occurrence is the posterior mandible. Solid tumors and distant metastases are associated with a poorer prognosis.
