Resumo: Brown tumor of hyperparathyroidism is a lesion composed of giant cells associated with primary or secondary hyperparathyroidism, which is invasive in some cases, but without neoplastic potential. A 36-year-old female patient presented with a complaint of painless swelling in the intra and extraoral region of the right mandible, with an approximate duration of four months. Imaging examination revealed an osteolytic and expansive area affecting the lateral region and angle of the right mandible, with significant thinning of the lingual and gingival cortices. An incisional biopsy was performed, and spindle cell proliferation with giant cells and hypercalcemia were observed. Laboratory tests were ordered to investigate endocrine disorders, which indicated hyperphosphatemia, hypercalcemia, and elevated parathyroid hormone, confirming the diagnosis of brown tumor of primary hyperparathyroidism. The patient was referred to the head and neck and endocrinology service and is being followed up with regression of the lesion.
