Resumo: Tooth extraction in patients treated by radiotherapy in the head and neck region remains a challenge. The objective of this work is to report the conservative clinical management of teeth with radiation caries (RadC). A 51-year-old woman with hydrocephalus was referred for treatment of teeth with RadC. Clinical examination revealed severe oral dryness, extensive caries involving the crown of the right and left lower canines. She had previously been treated for a myoepithelial tumor in the hard palate by surgery and adjuvant radiation therapy. The diagnosis of RadC was established based on the clinical history. The treatment invloved supragingival scaling, coronectomy, emptying and preparation of the root canals and filling with calcium hydroxide paste and coronary sealing with subsequent covering of the roots by the gums. Coronectomy is a therapeutic alternative for the treatment of teeth with indication for dental extraction in patients with high risk of developing osteoradionecrosis.
