Resumo: This report describes the challenge of imaging diagnosis of a bone finding in the mandible in a 26-yeard-old man, that has been observed for 6 years as bone sclerosis, which progressed painlessly without clinical findings. The patient did not present symptomatology, however, the most recent panoramic radiography showed a mixed image extending from tooth 46 to 47, suggesting the presence of a fibro-osseous lesion. Cone beam computed tomography showed a hyperdense image adjacent to the trabecular bone and intermingled between the roots of tooth 46 to the mesial root of 47, with expansion of the lingual cortical. At the request of the patient, partial resection of the lesion was performed and histopathological examination showed numerous bone trabeculae permeated by fibrous connective tissue and red blood cells, confirming the diagnosis of fibrous dysplasia. The radiological aspects confirmed the diagnosis of monostotic fibrous dysplasia. The case is under follow-up for 2 months.
