Resumo: Syphilis is a sexually transmitted infection caused by Treponema pallidum. The disease evolves in four stages: primary, secondary, latent and tertiary. In general, syphilis is diagnosed in the secondary phase because the lesions are more numerous, symptomatic. Lesions can affect the skin and oral mucosa. This paper will report the case of atypical oral lesions of syphilis in an adult man hospitalized for treatment of pulmonary tuberculosis. Intraoral examination revealed the presence of purplish nodular lesions whose initial clinical hypothesis was Kaposi´s sarcoma, since the patient had HIV/AIDS. The patient was submitted to incisional biopsy on the gingiva, because several ulcerated lesions had appeared in this region and hard palate. Histopathological examination and serological tests confirmed the diagnosis of syphilis. The patient was treated with intramuscular benzathine penicillin G. The patient presented a significant clinical improvement and there was regression of the oral lesions in 30 days of follow-up.
