Título: Immune profile of patients with oral squamous cell carcinoma (OSCC) with lymph node metastasis: a pilot study.

Resumo: Objective: To evaluate the different immunological patterns of immune populations in patients with lymph node metastasis (N+) in comparison with patients without lymph node involvement (N-) in OSCC patients. Study Design: In the prospective cohort, peripheral blood samples were collected from 21 OSCC patients in order to analyze the cellular immune profile by multiparametric flow cytometry. The retrospective cohort of 16 patients was designed to evaluate the tumor imune microenvironment through immunohistochemistry. Results: The first cohort consisted of 9 N- and 12 N+ patients. The analysis with N+ showed increased concentrations of NKT cells (CD3+CD56+), higher frequency of exhausted TCD4 cells (CD4+CD8-), immature dendritic cells (CD11c+), and non-classical monocytes. In the retrospective cohort evaluation, 8 N+ and 8 N- patients were included. N+ patients had higher immunexpression of dendritic cells (CD1a, CD83) and (PD-L1), consistent with the pro-tumor profile shown by the prospective cohort. Additionally, lower CD1a expression was associated with a priori in overall survival. Conclusion: Lymph node metastasis is a worse prognostic indicator for OSCC patients. Our preliminary data show that these patients have an exhausted immune profile. Thus, future studies may better clarify the role of exhaustion and whether these patients would benefit from any immunotherapy.
