Resumo: A 72-year-old male came to the clinic reporting a radiograph finding. The intraoral examination did not present any clinical alteration and the patient did not complain of pain. The radiographic examination evidenced an radiolucent, unilocular, well-delimited lesion with a radiopaque halo, located in the right body of the mandible, measuring approximately 2 cm of diameter. The diagnostic hypotheses were odontogenic keratocyst or residual cyst. An enucleation was performed. The histopathological evaluation demonstrated multiple fragments of a focally inflamed cystic wall of fibrous connective tissue lined by stratified squamous epithelium, three to nine cells layers thick, with a mostly orthokeratinized surface, prominent granular layer and a basal layer of cuboidal cells without reverse polarization lying flat over the basement membrane. Keratin production was abundant, forming multiple lamellated sheets inside the lumen. The final diagnosis was orthokeratinized odontogenic cyst. The patient remains under clinic and imaging control with no signs of recurrence.
