Resumo: A 40-year-old woman presented with a 3-month history of a painless ulcerated nodule on the upper lip. The patient reported the lesion started as a "darkened spot" which was treated by another clinician as herpes, without resolution. The dentist considered the possibility of an infectious process and performed an incisional biopsy. Microscopically, a proliferation of large lymphoid cells with marked pleomorphism, mitotic figures, eosinophils, and necrosis was observed. The neoplastic cells dissected muscle bundles, and some had a kidney-shaped nuclei. The immunohistochemical study showed positivity for LCA, CD30, CD4, CD8, CD68, Granzyme B, and Ki-67 index approximately 50%. CD20, CD79a, MUM1, ALK, and CD56 were all negative. In situ hybridization for EBV was negative. Three weeks after the surgical procedure, the patient returned with total regression of the lesion. According to the clinical, histopathological, and immunohistochemical features, the diagnosis of primary mucosal CD30-positive T-cell lymphoproliferative disorder was established.
