Resumo: Paragangliomas are well differentiated non-epithelial neoplasms derived from paraganglion cells of the autonomic nervous system. Most cases develop around the carotid body (40-60%) of female patients (1.7:1) in their 6th decade, with low metastatic potential. We contribute with an exceedingly rare case of paraganglioma affecting the posterior mandible of a 28-year-old male who presented an asymptomatic, ill-defined multilocular radiolucency with irregular margins in the mandibular ascending ramus. Medical history included a paraganglioma of the carotid body diagnosed 8 years earlier. An incisional biopsy was performed and microscopically, a zellballen architecture of chief cells with abundant pale cytoplasm and atypical nuclei, peripherally delimited by sustentacular cells, were observed in a highly vascular stroma. Mitotic figures were rare. Tumor chief cells were positive for synaptophysin and GATA3 while sustentacular cells were positive for S-100 and enolase. Ki-67 index was <1%. Metastatic paraganglioma may appear as an ill-defined radiolucency in the mandible.
