Resumo: The case reports a 58 year old female who presented with pain and periorbital edema, ophthalmoplegia, associated with the presence of purulent secretion in both nasal cavities. The facial panoramic radiography and tomography revealed a radiolucent lesion surrounded by a well-defined radiopaque halo in the right maxillary sinus, causing periorbital cellulitis and complete opacity of the frontal sinus. In the first approach, a transmaxillary sinusotomy using Caldwell-Luc access was performed, with cyst enucleation, wide resection of the maxillary sinus walls, and orbital access on the sinus roof with drainage of purulent secretion, complemented by frontal, ethmoidal, and sphenoidal sinusotomies on the right. Histopathological examination revealed a mucous cyst. In the second approach, a new drainage of periorbital purulent secretion was performed with supraorbital access and orbitotomy, with resection of synechiae on the right nasal cavity. The patient continued with antibiotic therapy and follow-up consultations.
