Resumo: Necrotizing sialometaplasia is an uncommon inflammatory condition of the salivary glands that can mimic malignancy, both clinically and microscopically. A 44-year-old woman, smoker, was referred for evaluation of a painful lesion in hard palate. Intraoral examination revealed a crater-like ulcer in the posterior region of the right hard palate, measuring approximately 2 cm. The patient reported no history of local trauma. Considering the diagnostic hypothesis of squamous cell carcinoma, an incisional biopsy was performed. Histopathologic examination revealed acinar necrosis, with preservation of the lobular architecture, and squamous metaplasia of the salivary ducts. The parakeratinized overlying epithelium exhibited areas of pseudoepitheliomatous hyperplasia. The final diagnosis was necrotizing sialometaplasia. The postoperative course was uneventful. At 8 months of clinical follow-up, there were no signs of recurrence. Necrotizing sialometaplasia is a benign, self-limiting condition that can simulate a malignant process and, therefore, its correct diagnosis is crucial to avoid unnecessary radical surgery.
