Resumo: Osteoradionecrosis (ORN) is one of the most serious complications of head and neck radiotherapy and its treatment remains challenging. Studies suggest the use of platelet and leukocyte-rich fibrin membranes (L-PRF) for management and prevention of ORN. A 68-year-old male patient, diagnosed with tonsillar squamous cell carcinoma, underwent chemotherapy and radiotherapy. After three years, he presented periapical lesion of teeth 46 and 47, which underwent endodontics. As the bone lesion continued to progress, he was prescribed pentoxifylline and tocopherol, and preserved for more than 1 year. After this period, he reported spontaneous avulsion of tooth 46 evolving with bone exposure and pain. Exodontia of tooth 47, bone debridement, and use of L-PRF membranes were indicated. Anatomopathological examination confirmed ORN. Six months after surgical procedure satisfactory healing was observed with no signs of infection or pain. Therefore, L-PRF can be a helpful option for ORN treatment.
