Resumo: A 33-year-old male patient attended the hospital´s outpatient clinic with an asymptomatic swelling in the left mandible, with more than 10 years of evolution. Imaging examination showed an expansive and osteolytic multilocular lesion affecting left mandibular body, angle and ramus. An incisional biopsy was performed. Histopathological analysis revealed a fragment of malignant neoplasm characterized by proliferation of mucous, intermediate, and squamoid cells. Architectural configurations included solid areas and cystic spaces partly lined by mucous cells with abundant mucinous cytoplasm, displaying intracitoplasmatic alcian blue and periodic acid-Schiff staining with diastase resistance. The diagnosis was low-grade mucoepidermoid carcinoma (MEC). The patient was referred to head and neck surgery, where a composite hemimandibulectomy was performed. The diagnosis of central intraosseus MEC was confirmed in the surgical specimen. No signs of relapse were observed in 1-year follow-up.
