Resumo: A 35-year-old female complains of pain in the right side of the mandible on the first molar region. Intraoral examination showed a slight swelling in the right lingual mandibular region, tooth 46 extruded and mobility grade III in tooth 47. Panoramic radiograph revealed a multilocular radiolucent area involving teeth 46 and 47, with well-defined borders. Aspiration puncture was negative, and both teeth involved were vital. Cone-beam computed tomography showed dislocation of the mandibular canal and rupture of the lingual cortical bone. An incisional biopsy was performed, and histopathological analysis showed a fibrous stroma with epidermoid, mucinous, and intermediate cells forming cystic structures sometimes filled with mucus. The diagnosis was low-grade intraosseous mucoepidermoid carcinoma, an atypical malignant neoplasm. Its prognosis is related to the histopathological grade and tumor stage. However, 5-year survival rates for low-grade lesions are up to 95%. The patient was referred to the Oncology Hospital for treatment.
