Resumo: This case refers to an 11-year-old female patient, asymptomatic, in a periapical radiographic finding of tooth 46, a well-defined radiolucent area was observed in its apical region. An excisional biopsy was performed, under the diagnostic hypothesis of an inflammatory follicular cyst, and, during the procedure, the presence of purulent secretion was noted. Microscopically, fibrous connective tissue with a capsular appearance was observed, covered by a few layers of epithelial cells. In the fibrous capsule, there were lymphoplasmacytic and neutrophilic inflammatory infiltrate, and pseudoxanthomatous cells. Based on the radiographic, epidemiological, and microscopic characteristics, the diagnosis was mandibular buccal bifurcation cyst, an uncommon entity of odontogenic and inflammatory origin. It develops on the buccal face of the newly erupted first or second permanent mandibular molar, from infections of the periodontal tissues, and can result in swelling, pain, and buccal tipping of the tooth, requiring surgical intervention.
