Resumo: A 67-year-old white female patient went to the Oral Diagnosis Center with heterogeneous white patches on the palate, buccal mucosa, and dorsal tongue. Incisional biopsy of the palatal lesion revealed hyperkeratinized stratified squamous epithelium with broad projections and basal layer polarization, characterizing a moderate epithelial dysplasia. Based on these findings, the patient was diagnosed with proliferative verrucous leukoplakia (PVL), as she presented 3 major and 3 minor criteria recommended for diagnosis. During the previous 11 years of follow-up, this patient had reticular white lesion in bilateral buccal mucosa and desquamative gingivitis, histologically characterized by an interface mucositis pattern. Both clinical and histopathological aspects led to the diagnosis of oral lichen planus (OLP). The progression of OLP lesions to PVL is a controversial topic in the literature. However, these clinical characteristics suggest a greater risk of malignant transformation, highlighting the need for regular clinical follow-up and biopsies.
