Resumo: The aim of this study is to report a case of a patient diagnosed with oral lichen planus (OLP), proliferative verrucous leukoplakia (PVL) and lip cancer. Male patient, 71 years-old, sought a stomatology service with an ulcerated lesion on the lower lip. On clinical examination, a whitish plaque resistant to scraping was observed on the tongue and an ulcer measuring about 2 cm on the lower lip. Histopathological analysis confirmed OLP lesion in the tongue and squamous cell carcinoma in the lip. The patient was referred, and resection of the lip tumor was performed. The patient lost follow-up and returned after three years with new white plaques on the buccal mucosa, tongue, and hard palate, which already showed moderate epithelial dysplasia, with a clinical diagnosis of PVL. Considered a high-risk patient, frequent follow-up and new biopsies confirmed the presence of epithelial dysplasia, but without signs of malignant transformation so far.
