Resumo: A 60-year-old female was referred to manage a secretory carcinoma (SC) with compromised margins previously excised in another service. The patient reported an ulcerated, bleeding, painful lesion on the left soft palate for 20 years with progressive and exuberant growth in the last two years. Computerized tomography revealed lesion infiltration extending the oral cavity to the pharyngeal space. The patient underwent surgical treatment and an anatomopathological examination showed infiltrative epithelial neoplasm-presenting cells with a large vacuolized and eosinophilic cytoplasm. The nuclei were vesicular with prominent nucleoli. The tumor cells are arranged in a pattern composed of solid, microcystic, and follicular structures with an eosinophilic luminal secretion of a bubbling appearance. Figures of mitosis were frequent and all margins were compromised. The neoplastic cells showed positivity for S100, mammaglobin, and vimentin thus establishing the diagnosis of SC. The patient is currently being followed up for further therapeutic intervention.
