Resumo: A 57-year-old man with a 74-pack-year smoking history complained of hoarseness, dry cough, dysphagia, and weight loss over two months. Nasovideolaryngoscopy revealed left vocal fold paralysis, and the oral examination showed a submucosal nodule in the left tonsil. An incisional biopsy was conducted, and a chest computed tomography (CCT) was ordered to investigate the aortic-pulmonary window and laryngeal nerve insertion. The histological examination of the oral nodule revealed a poorly differentiated tumor consisting of large, irregularly shaped cells separated by a fibrovascular stroma, which were positive for CD56, chromogranin, synaptophysin, and EMA. The CCT revealed an extensive endobronchial lesion in the left lower lobe superior segment, and the morphological and immunohistochemical (IHC) analysis of the biopsy matched the oral lesion. The diagnosis of metastatic large-cell neuroendocrine carcinoma of the lung in the oral mucosa was confirmed based on the clinical, morphological, and IHC findings. The patient was referred for chemotherapy and remains under treatment.
