Resumo: Acute lymphoblastic leukemia (ALL) is the most common malignancy in children and adolescents with signals and symptoms correlated with bone marrow failure, such as pancytopenia. Individuals may experience recurrent infections associated with a decreased white blood cell count. Case report: a 15 years old female with ALL in maintenance period, attended to the hospital emergency complaining about pain and gum bleeding for four days. Patient´s exams revealed significant leukopenia (1050 cells/mm³), anemia (hemoglobin 9,7g/dL) and thrombocytopenia (57.000 platelets). She was referred to the dental team. Intra-oral exam showed spontaneous bleeding, ulceration and necrosis in the gingival surfaces. Clinical diagnosis was necrotizing ulcerative gingivitis (NUG). Empiric treatment with tazobactam and clindamycin, tooth brushing with a solution of chlorhexidine 0.12%, photodynamic therapy and daily scraping were applied. On hospital day 5, the patient denied gingival pain and was discharged home.
