Resumo: A 47-year-old male patient presented with a 5-month history of headaches and diplopia. No comorbidities were reported. MRI showed an expansive image affecting the ethmoid sinus and nasal cavity bilaterally (56x40x37mm), extending intracranially and into the orbits. Incisional biopsy analysis revealed a sinonasal undifferentiated carcinoma (T4aN0M0). Induction chemotherapy with TPF protocol added to CDDP cycles associated with high-dose radiotherapy were performed, with subsequent complete remission. After 45 months of follow-up, the patient presented a painful midfacial swelling. CT scan revealed a destructive image extending from the maxillary sinus to the intracranial region, with multiple regional lymphadenopathies. Microscopic examination showed a poorly differentiated tumor with large round cells. An immunohistochemistry panel revealed positivity for epithelial markers and a diffuse expression of neuroendocrine markers (CD56 and synaptophysin), a feature not previously observed in the initial lesion. Diagnosis of large cell neuroendocrine carcinoma was established, and surgical-oncologic treatment was performed with subsequent follow-up.
