Resumo: A 6-year-old boy with a swelling in the left mandible of 18-month evolution is presented. According to his mother, he received antibiotics without any positive results. Later teeth 74 and 75 were extracted. The swelling persisted one month after the extractions, and the patient manifested mild pain only during chewing. At this moment, panoramic radiography showed a rounded radiolucent lesion involving the complete tooth germ of 74 and partially the germ of 75. It appeared with a definite cortical line. Computer axial tomography showed a hypodense, expanded lesión with partial destruction of the vestibular cortical bone.The microscopic study revealed spindle cell proliferation intermixed with plasma cells, histiocytes, fibroblasts, eosinophils, and lymphocytes. Staining for SMA was positive in the cytoplasm of spindle cells. Ki67 was only positive in inflammatory cells. The final diagnosis was an inflammatory myofibroblastic tumor. Curettage was the treatment. Follow-up after one year of treatment is shown.
