Resumo: Computer-aided design (CAD) has become standard in mandibular reconstruction.
CAD was used in 6 patients since January 2021, all for mandibular reconstruction with fibula free flap (3 after resection for squamous cell carcinoma and 3 for mandibular osteoradionecrosis). In one patient the cutting guides were provided with multiple different cutting levels. In another case, the flap was fabricated with dental implants.
Total intraoperative time using CAD was reduced when comparing with our previous experience. Multiple different cutting levels permitted more liberty for resection of cancer cases. Good contact of bony segments was obtained in all patients. In one case the cutaneous island of the fibula flap did not match the area needed to cover. In the case of dental implants, we had a complication of mismatching in dental oclusion.
CAD helped our team on intraoperative time and bone reconstruction planning, although other variables have emerged.
