Resumo: Ameloblastic carcinoma is a rare malignant epithelial neoplasm. A 52-year-old white male patient presented with an expansive lesion in the posterior region of the right mandible. On clinical examination, a purplish nodule in the region of the body and right mandibular ramus, painful on palpation, was noted. Computed tomography scans showed an unilocular hypodense lesion, in the posterior mandibular region with lingual expansion and areas of bone fenestration. Incisional biopsy was performed with a histopathological report of ameloblastoma. Surgical excision of the lesion was planned, mandibular resection specimen being sent to our pathology service where it was possible to observe a malignant epithelial odontogenic neoplasm vaguely resembling ameloblastoma and characterized by proliferation of cells arranged in multiple patterns like bone trabeculae, densely cellular solid islands, cords, and nests with a histopathological report of ameloblastic carcinoma. Patient was referred to the oncology service to continue the treatment.
