Resumo: Secondary lymphedema in head and neck cancer (LSCCC), which occurs as a result of direct tumor invasion, radiation therapy or surgical resection that disrupts lymphatic drainage, is poorly studied, however its prevalence is estimated at 75%. LSCCC can result in infections, cellulitis and long-term cosmetic, functional and psychosocial complications that are often irreversible. Complete decongestive therapy (CDT) is the standard treatment and others such as microvascular surgery, liposuction, drugs, acupuncture and photobiomodulation are reported. We present a 74-year-old female patient with a history of epidermoid carcinoma of the floor of the mouth with 3 events during 2017, 2020, 2021 treated with surgery, lymphadenectomy and IMRT 70 Gy+Cisplatin 100 mg/m2. At 2 months she developed bilateral facial LSCCC treated with photobiomodulation: diode laser 660nm- 808nm - 10 sessions, fluence of 18 joules per weekly application point. There was a good clinical response to treatment with no recurrence of LSCCC.
