Resumo: A female patient, 14 years old, complained of a painful ulcer present on the tongue for 6 months. She reported previous treatment with Omcilon-A Orabase, but without success. Intraoral examination revealed a 4.5 cm ulcer with raised edges and hardened on the left lateral border of the tongue. Extra-oral physical examination revealed a hardened and enlarged level II left cervical lymph node, painless on palpation. Under the diagnostic hypothesis of squamous cell carcinoma (SCC), an incisional biopsy was performed. Microscopically, sheets and nests of neoplastic epithelial cells were observed, exhibiting nuclear pleomorphism, hyperchromatism, atypical mitoses, and horny keratin pearls, confirming the diagnosis of SCC. The specimen was positive for p16 but negative for high-risk HPVs. The tumor was classified as stage III (T3N1Mx) and the treatment included hemiglossectomy, neck dissection, and forearm freeflap reconstruction. The patient underwent adjuvant radiotherapy and chemotherapy and is disease-free after 26 months of follow-up.
