Título: TREATMENT OF EXTENSIVE OSTEORADIONECROSIS OF MANDIBLE: CASE REPORT

Resumo: Male patient, 65 years old, history of radiotherapy to treat a laryngeal Scamous Cell Carcinoma 6 years earlier. Extration with osteotomy was performed in the mandible and after that began to feel intensive pain. Physical examination showed an intensive bone exposure with purulent secretion and the image exame showed a diffuse bone loss without fracture. Weekly lasertherapy and aPDT with 0,1% methylene blue in intra and extraoral points were iniciated with antibiotic (Amoxicillin with Clavulanate and Metronidazole). After 9 months, the patient had an extraoral fistula and a biopsy was performed, in order to confirm the diagnosis of osteoradionecrosis with gingival hyperplasia. aPDT and autologous leukoplatelet fibrin were used during the biopsy. An antibiogram was collected and it was started Ciprofloxacin with Fluconazol, with weekly aPDT and lasertherapy. In 6 months we had a bone reparation and a significant improvement of intra and extra oral exposures, along with pain reduction.
