Resumo: Chronic ulcerative stomatitis (CUS) is a rare mucocutaneous disease, which affects mucous membranes and skin, leading to the development of recurrent and remitting erosive/ulcerative lesions, which may be similar to other immune-mediated diseases, especially Lichen Planus, leading to erroneous diagnoses. A 67- year-old female patient, diagnosed with oral lichen planus in 2012, presented ulcerated and painful oral lesions, which did not respond to previous treatments (topical clobetasol, dexamethasone, chamomile tea, and systemic methotrexate). A new biopsy associated with direct immunofluorescence was performed, showing positivity for fibrinogen, and negativity for IgM, IgG, IgA and C3. The patient underwent a new therapeutic protocol with oral hidroxicloroquine (200mg per day) for one month, wherein exhibited improvement in oral lesions and symptomatology. In conclusion, association of the clinical presentation with laboratory findings and the therapeutic results, the final diagnosis was CUS. The patient remains with the treatment with no other symptomatic crisis.
