Resumo: This study reports a case of bisphosphonate-associated osteonecrosis of the jaw (ONJ) in an oncology patient. The case involves a patient diagnosed with breast cancer in 2007 who underwent radiotherapy and chemotherapy. In 2020, the patient developed metastasis in the lung and bones. She received chemotherapy treatment combined with Zoledronate. Upon intraoral examination, an area of exposed bone was observed in the region of tooth 17, which had been extracted for dental implant placement. The patient reported local pain and, radiographically (through panoramic and computed tomography), areas of bone sequestration were observed. The proposed treatment was decontamination with 0.12% chlorhexidine, Photodynamic Therapy (with methylene blue), oral hygiene instruction, and the PENTO protocol (pentoxifylline, tocopherol, and amoxicillin). After five weeks, the patient showed improvements in symptoms and a reduction in bone exposure. The case highlights the importance of palliative care in reducing pain and improving the patient´s quality of life.
