Resumo: We report a case of a 14 years old melanoderma male who presented a well corticated unilocular radiolucent lesion associated with the third molar crown and taking much of the right maxillary sinus, discovered in a routine exam. A drainage rigid dispositive was placed as well as an incisional biopsy was performed, which revealed an Odontogenic Keratocyst diagnosis. The treatment plan consisted in decompression and weekly rising with saline solution and rifamycin, which showed a 50% decrease of the lesion size after 10 months. Due to incomplete resolution, it was followed by enucleation and curettage along with associated tooth removal and application of Carnoy´s solution. 1 year postsurgery tomography showed a bone neoformation with no sign of reoccurrence. Enucleation combined with adjunctive therapy, such as the application of Carnoy's solution or decompression before enucleation, has showed an important decrease of reoccurrence rates when compared to enucleation alone.
