Resumo: A 40-year-old man presented with multiple translucent and hemorrhagic papules distributed in gingival mucosa. These lesions were preceded by a single hemorrhagic papule in palatine interdental papilla that spread to vestibular papilla and marginal gingiva over the period of two months. A biopsy was performed under the clinical hypothesis of Wegener´s granulomatosis. The histopathology showed marked epithelial exocitosis and pseudoepitheliomatous hyperplasia. An intense, mixed inflammatory infiltrate composed by lymphocytes, histiocytes and polymorphonuclear cells along with extravasated red blood cells were identified in lamina propria. Laboratorial tests such as ANCA serology, creatinine dosage, urine sediment, as well as abdominal computed tomography scan were performed and allowed the final diagnosis of superficial Wegener´s granulomatosis. Patient was referred to a reumathologist and has been treated with prednisone for 6 months. Gingival lesions showed partial involution.
