Resumo: Medication-related osteonecrosis of the jaw is a rare and severe condition that negatively impacts the lives of affected patients. Its multifactorial pathophysiology has not yet been fully elucidated. As new preclinical and clinical studies are published, more evidence collaborates to assemble the intricate puzzle that is this pathology, but new questions also arise. Objective: The present study aimed to sum up the main hypotheses raised so far for the pathophysiology of Medication-related osteonecrosis of the jaw. Study Design: A narrative review of the literature was performed in Pubmed, Scopus, Embase and Web of Science databases. Results: The main hypothesis considers the disturbance of bone turnover caused by the inhibition of osteoclast activity, resulting in accumulation of bone microfractures. Oral infection and inflammation, the antiangiogenic effect and soft tissue toxicity of bisphosphonates, as well as anti-lymphangiogenesis, dysfunction in immunity and genetic predisposition seem to play an important role in the etiopathogenesis of the disease. Conclusion: Understanding the mechanisms behind Medication-related osteonecrosis of the jaw is essential to guide research and testing of new preventive and treatment strategies.
