Resumo: The first purpose of this case series was to describe a technique for fabricating a custom prosthetic mouthguard to manage neuropathological mastication (NPM) in critically ill patients from intensive care units. Such device comprises a hard elastomeric impression material covered by an acrylic interocclusal layer, maintaining the patient's mouth open due to the elevated contact on its posterior region. It is a straightforward and low-cost appliance that has the potential to reduce the occurrence of complications, such as self-inflicted traumatic lesions into oral cavity or cardiopulmonary arrest induced by hypoxia after the obliteration of orotracheal tubes, for example. The present article adds information regarding the profile of individuals who develop complications resulted from NPM. The second purpose was to highlight the positive impact that prosthodontists have in ICU units since prosthetic handling enhanced medical care. Oral lesions were managed, sedation was interrupted and mechanical ventilation was weaned after mouthguard installation.
