Resumo: A 16-year-old male patient attended the oral and maxillofacial surgery service, complaining of a painless swelling in the maxilla. Clinically, there was a volume increase in the region of the upper left canine, causing facial asymmetry. The cone beam computed tomography showed a radiolucent well-delimited lesion with approximately 10mm in diameter. Based on clinical and image appearance, an excisional biopsy was performed. The microscopy revealed a fibrous capsule covered by odontogenic epithelium with extensive intraluminal projection, showing the presence of plexiform cords, ductiform structures, and formations similar to rosettes. Correlating clinical, imaging, and microscopic aspects, the final diagnosis was extrafollicular adenomatoid odontogenic tumor, one of the three variants of this type of unusual benign neoplasm that originates from odontogenic epithelial cells, commonly associated with females, especially in young people. Its slow and progressive growth explains the low recurrence rate and justifies conservative surgical treatment.
