Resumo: Non-Hodgkin's lymphomas (NHL) are a group of disorders involving malignant monoclonal proliferation of lymphoid cells that are relatively uncommon in the oral mucosa. A 73-year-old woman was referred for evaluation of a painful swelling on the palate. Medical history revealed rheumatoid arthritis and osteoporosis treated with Methotrexate and folic acid. Intraoral examination showed a submucosal swelling, hard to palpation, on the soft palate. The provisional diagnosis was a salivary gland tumor, mainly adenoid cystic adenocarcinoma due to the pain complain. After incisional biopsy, histological examination showed a moderate differentiated neoplasia, suggesting a lymphoproliferative disorder. The neoplastic cells were immunopositive for CD20, CD10, bcl-2, bcl-6 and a Ki67 index of 60-70%, providing the diagnosis of large B cell NHL. The patient remains under chemotherapy. Although NHL is rare in the oral cavity, it should be considered as differential diagnosis of oral submucosal swellings suggestive of salivary gland tumors.
