Resumo: We report case of a 36-year-old woman with a painful lesion on the hard palate during 3 weeks. Her medical history was positive for Systemic Lupus Erythematosus (SLE), fibromyalgia, and hypothyroidism. She reported a vaginal ulcer 8 months earlier, with regression. Intra-oral examination had revealed multiple mucous patches with an erythematous halo in hard palate and upper and lower labial mucosa. VDRL (Venereal Disease Research Laboratory) and incisional biopsy of the palate lesion with direct immunofluorescence analysis (DIF) was performed due to the diagnostic hypothesis of secondary syphilis and oral manifestation of SLE. Histopathological examination revealed focal fibrin-coated epithelial ulceration with polymorphs, and moderate and diffuse intercellular edema with non-specific DIF. VDRL and fluorescent treponemal test was positive, with a final diagnosis of secondary syphilis. Treatment was given with total dose of 7.2 MU benzathine benzylpenicillin intramuscular. Oral lesions improvement was reported 2 weeks after end of this treatment.
