Resumo: The dental surgeon who acts in a hospital setting has an important hole in the aid of diagnosticand management of oral manifestation of hematologic disorders, such as ImmuneThrombocytopenic Purpura (ITP). A 23-year-old woman sought care in the EmergencyDepartment of a University Hospital due to a gingival bleeding condition. The bleeding startedafter a scaling procedure performed a month ago by the Primary Health Care (PHC) dentist. Onphysical examination, swelling and widespread purplish gingival hematoma was observed, aswell as actively bleeding in free and attached gingiva. Local hemostasis was achieved withtranexamic acid rinses. The hematological examination (HCT 16%; Hb 4,9% and PLT 2000)suggested iTTP, an autoimmune disease, characterized by low platelet count, but which usuallyhas self-limited bleeding. The differential diagnosis with leukemic infiltrate was considered. Thedental surgeon can contribute to the diagnosis and treatment of the oral manifestations ofITP.
