Resumo: Oral squamous cell carcinoma (OSCC) is the most common malignancy of the oral cavity, usually affecting smoker individuals over 50 years of age. Multiple primary OSCCs are uncommon in clinical practice. The aim of this report was to describe three synchronous OSCCs. A 68-year-old female, a non-smoker and non-drinker, was referred to our service for evaluation of a painful granulomatous ulcer on the floor of mouth that had appeared approximately two months earlier. Intraoral examination revealed two other similar lesions, on the border of the tongue and lower gingiva, both on the right side. The diagnostic hypotheses were paracoccidioidomycosis and OSCC. An incisional biopsy was performed, and a microscopic examination indicated a diagnosis of OSCC in both lesions. For treatment, the patient was referred to the oncology team and was still under follow-up at this writing. This case shows a complete diagnosis performed by dentistry, which led to appropriate treatment.
